Yes — new patients complete registration, medical and dental history, a financial policy and a consent form when they arrive. Having your insurance, medication and physician details to hand makes it quicker.
Tell us. Our dental history form asks exactly that, because it changes how we plan your visit. Sedation is available for patients who find treatment difficult.
